ABSTRACT

Objective: To explore the impact and mechanism of disease management of caregivers on self-management behaviors among patients with chronic heart failure. Methods: A total of 231 dyads of outpatient chronic heart failure patients and their caregivers were assessed by caregivers contribution to self-care of heart failure index (CC-SCHFI), self-care of heart failure index (SCHFI) and Atlanta heart failure knowledge test (AHFKT-V2). IBM SPSS 22.0 was used to conduct the paired t test and Pearson correlation analysis. LISREL 8.80 was used to conduct the path analysis. Results: There were significant differences between caregivers and patients in maintenance behavior (P=0.015), management behavior (P=0.023) and self-efficacy (P=0.001). There was no correlation between caregiver self-efficacy and patient heart failure knowledge and skill, but other variables were positively correlated, and r value ranged from 0.129 to 0.575 (P<0.05). Patient self-management behaviors were directly influenced by caregiver disease management, patient heart failure knowledge and skill and patient self-efficacy, and indirectly influenced in part by caregiver disease management through patient heart failure knowledge and skill and patient self-efficacy. Conclusion: Caregiver disease management not only can directly influence patient selfmanagement behaviors, but also can indirectly influence patient self-management behaviors through patient heart failure knowledge and skill and selfefficacy. Health care workers should carry out family-centered health education, and help caregivers actively participate in disease management of patients with heart failure; for those patients whose caregivers can't change their disease management behaviors, health care workers should also improve their selfmanagement behaviors by improving patients' heart failure knowledge and skill and self-efficacy.

ABSTRACT

<p><b>OBJECTIVE</b>To demonstrate the clinical outcomes of maximal medical therapy(MMT) and functional endoscopic sinus surgery+ maximal medical therapy (FESS+MMT) for moderate to severe chronic rhinosinusitis without nasal polyps (CRSsNP) in China, to provide clinical evidence for treatment recommendation.</p><p><b>METHODS</b>A prospective multicenter case control study consisting of 90 strictly selected CRSsNP patients were carried out by ENT Hospital of SUN Yet-sen university and 7 other university affiliated hospitals across China from March 2011 to October 2012. All patients were selected to MMT group or FESS+MMT group followed by 3 months treatment and 6 months follow up. Treatment efficacy evaluation indicators included improvement of visual analogue scale (VAS) score, quality of life, CT score and post-operative nasal endoscopic evaluations. SPSS16.0 software was used to analyze the data.</p><p><b>RESULTS</b>All patients enrolled complied with moderate to severe CRSsNP evaluation criteria. There were no significant differences found in the baseline data between two groups (P > 0.05). (1) At the 3 time points (pre-treatment, 3 months and 6 mongths after treatment) , VAS overall symptom score in MMT group were 6.52 ± 2.61, 2.66 ± 2.10, 2.40 ± 1.56, significant differences were found among them (t value were 2.083 and 2.295, both P < 0.05); in FESS+MMT group the values were 6.99 ± 2.70, 0.95 ± 0.84, 0.60 ± 0.81, significant differences were found among them (t value were 3.582 and 5.196, both P < 0.05); SNOT-20 score in MMT group were 38.61 ± 17.36, 18.59 ± 14.04, 18.40 ± 8.91, significant differences were found among them (t value were 2.737 and 2.657, both P < 0.05); in FESS+MMT group the values were 38.21 ± 19.61, 5.94 ± 5.01, 2.65 ± 2.31, significant differences were found among them (t value were 3.247, 3.319, both P < 0.05). (2) FESS+MMT group relative to the MMT group in VAS overall symptom score and quality of life improvements appeared earlier and were more pronounced. (3) Overall treatment efficacy showed that in MMT group: complete control 14 cases (30.4%), partially control 31 cases (67.4%), uncontrolled 1 cases (2.2%); In FESS+MMT group: complete control 17 cases (38.6%), partially control 26 cases (59.1%), uncontrolled 1 cases (2.3%). (4) Patients' satisfaction survey showed that the number of the patients who were very satisfied with the efficacy in the FESS+MMT group were 2 times higher than the MMT group.</p><p><b>CONCLUSIONS</b>(1) For moderate to severe CRSsNP, both MMT and FESS+MMT treatment can effectively control the overall symptoms and classified symptoms, reduce CT scores and significantly improve the quality of life, the ineffective rate is less than 5%. (2) FESS+MMT group in terms of improving symptoms and the onset time are better than MMT group, especially in improving the stuffy nose, head and face fullness, and mental and physical symptoms are better than MMT group. (3) FESS+MMT group showed better results in patient satisfaction survey compared to the MMT group. Therefore for moderate to severe CRSsNP patients, FESS+MMT therapy could be recommended as the preferred treatment.</p>

ABSTRACT

Our work focuses on the quality control and structural identification of Photocyanine as a cancer therapeutic photosensitizer. Photocyanine is a mixture which contains four ZnPcS2P2 type substituted Phthalocyanine isomers. In order to obtain the single component from Photocyanine, the mixture of four isomers possessing the similar structures and chemical property had been isolated and purified. An HPLC method with a mixture of methanol-acetonitrile-ion-pair buffer as the mobile phase was applied to isolate the four isomers by means of a semi-preparative C18 column. To remove the salts which were mixed in the preparative product, a SPE C18 column was used to separate the salts by elution with water and then the marker component was eluted by methanol. Subsequently, a column of Sephadex LH-20 gel was applied to elute the crudes with methanol to desalination. The purity of the isolated compound was measured by TLC and four different isomers of phthalocyanine were obtained. The chemical structures of them were elucidated by 1H NMR spectra, gCOSY and NOE1D. An HPLC-DAD method was developed for simultaneously determination of four major isomers in Photocyanine with a C18 column (Grace Smart, 150 mm x 4.6 mm ID, 5 microm). The separation was carried out with a gradient program at a flow rate of 1.0 mL x min(-1). The mobile phase was a mixture of acetonitrile and ion-pair buffer (0.01 mol x L(-1) hexadecyl trimethyl ammonium bromide and 0.01 mol x L(-1) potassium dihydrogen phosphate, adjusted the pH value to 6.8 with potassium hydroxide solution). The resolution values of four isomers were 2.5, 1.20, 1.33, and 1.8. Linear regression analysis for four compounds was performed by the external standard method. Four constituents were linear in the concentration range of 0.005 to 10 microg. The values of relative standard deviation (RSD) of intra-day were 0.12%, 0.66%, 0.99%, and 1.21%, respectively. The limits of detection for four compounds were 15 ng, 20 ng, 12 ng, and 25 ng, respectively. This method was simple, accurate and reproducible. The developed method can be successfully applied to analyze isomers in Photocyanine.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the effects of nasal mometasone furoate on moderate to severe allergic rhinitis.</p><p><b>METHODS</b>Patients with moderate to severe allergic rhinitis were enrolled and received mometasone furoate nasal spray 200 microg once daily for four weeks. Four hundreds and sixty-three patients completed the study. We weekly interviewed the patients to evaluate the symptoms, and the affection of disease on night sleep and daily life. Mini Rhinoconjunctivitis Quality of Life Questionnaire (Mini-RQLQ) and Nocturnal Rhinoconjunctivitis Quality of Life Questionnaire (NRQLQ) were used to evaluate the quality of life.</p><p><b>RESULTS</b>The individual and total symptomatic scores of week 1, week 2 and week 4 decreased compared with baseline. The scores of Mini-RQLQ and NRQLQ of week 1, week 2 and week 4 decreased compared with baseline too. The scores of the next visit were significantly lower than those of the previous visit (P < 0.01).</p><p><b>CONCLUSIONS</b>A four-week administration of mometasone furoate nasal spray can effectively treat allergic rhinitis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To construct the recombinant plasmid and standard curve for detection of epidermal growth factor receptor variant III (EGFRv III) by real-time quantitive polymerase chain reaction (PCR) and establish the RT-PCR assay for accurate detection of EGFRv III.</p><p><b>METHODS</b>To amplify the DNA sequences of exon 1 and exon 8 to exon 9 of EGFR separately and fuse the above sequences by overhang extension PCR. Then constructing the recombinant plasmid of EGFRv III by highly specific primers with the fused sequence as its template. The recombinant plasmid of EGFRv III was then sent for sequence analysis. The gradient diluted recombinant plasmid were used as the template to amplify the EGFRv III by RT-PCR with TaqMan probe and then applied it in 32 laryngeal carcinoma tissues.</p><p><b>RESULTS</b>The recombinant plasmid of EGFRv III was successfully constructed. The gradient diluted recombinant plasmid could be used in RT-PCR for quantitative analysis of EGFRv III. In all, 5 tumor samples expressing EGFRv III were detected, with its positive rate of about 15.6%. The expression of EGFRv III was tumor specific with only one exception for extraordinarily low expression of EGFRv III in macroscopically normal laryngeal mucosas adjacent to the tumor.</p><p><b>CONCLUSIONS</b>RT-PCR with TaqMan probe was good at sensitivity, specificity, quantification and linear function. It can be a standard method for quantitative detection for EGFRv III. Expression of EGFRv III was detected in laryngeal carcinoma tissues with relatively low level. Whether EGFRv III was a suitable target for biological therapy in laryngeal carcinomas remained to be discussed.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To establish a method of quantitative analysis of epidermal growth factor receptor(EGFR) mRNA in laryngeal carcinoma by reverse transcription-real time polymerase chain reaction with TagMan probe and evaluate its applicable value.</p><p><b>METHODS</b>The technique of reverse transcription-real time polymerase chain reaction with highly specific primers and probe was applied. And with GAPDH as internal standard, EGFR mRNA in 32 laryngeal carcinoma tissues, compared with their own macroscopically normal laryngeal mucosa tissues adjacent to the tumors, were quantitatively examined. The recombinant plasmid of EGFR was constructed, and then sequenced.</p><p><b>RESULTS</b>The recombinant plasmid of EGFR was successfully constructed. And the mRNA index of EGFR in 32 laryngeal carcinoma tissues in terms of M (QR) was 0.025 (0.076), which was higher than 0.008 (0.027) in their own macroscopically normal laryngeal mucosa tissues adjacent to the tumors, P value is 0. 007.</p><p><b>CONCLUSIONS</b>The method of reverse transcription-real time polymerase chain reaction with TagMan probe is quite objective, precise and high throughput in the application for the mensuration of EGFR mRNA in laryngeal carcinoma.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To establish a HPLC method for determination two constituents in bark of Paeonia Suffuticosa.</p><p><b>METHOD</b>The reversed phase HPLC system consisting of an Alltima ODS column (4.6 mm x 150 mm, 5 microm) and a mixture of water-THF-methanol-HAc (60:20:20:0.05) as the mobile phase was used. The flow rate was 0.8 mL x min(-1) and UV detection was set at 274 nm.</p><p><b>RESULT</b>The assay displayed good linearity over the concentration ranges of 0.06-1.0 microg (r = 0.999 9, gallic acid) and 0.16-2.58 microg (r = 0.999 9, paeonol) respectively. The average recoveries (n = 9) of gallic acid and paeonol were 98.6% (RSD = 3.0%), 98.2% (RSD = 2.5%), respectively. The samples were extracted with methanol for 24 h bu maceration.</p><p><b>CONCLUSION</b>The method is simple, accurate and can be used for the quality study of bark of P. suffruticosa.</p>
